This form requires students to explain why they didn't do their class work or homework instead of simply not turning it. It is great documentation to support students who earn failing/low grades due to missing work. These forms also can be used during parent conferences to show the reasons students are giving for not doing their work. It puts the responsibility back on the students where it belongs.
